The spine is built from 33 bones called vertebrae, organized into regions. The top seven — identified as C1 through C7 — form the cervical spine, or neck. Below the neck are 12 thoracic vertebrae (T1–T12), which anchor the ribs, followed by five lumbar vertebrae (L1–L5) in the low back, then the sacrum and coccyx.
Between each vertebra sits a spongy intervertebral disc that acts like a shock absorber, cushioning the bones as the spine twists, bends, and jolts. Each disc has a tough outer ring, the annulus fibrosus, surrounding a gel-like center called the nucleus pulposus — often compared to a jelly donut. Surrounding the bones and discs are the muscles, ligaments, tendons, blood vessels, and nerves that give the neck its remarkable flexibility.
That flexibility comes at a cost. Unlike the lower back, the cervical spine has comparatively little muscular support, yet it must constantly bear the full weight of the head — roughly ten to twelve pounds. When a crash, fall, or blow forces the head to snap suddenly forward, backward, or sideways, the neck’s limited support structure often cannot absorb the force, and injury results.
“Whiplash” is the everyday term for a hyperflexion-hyperextension injury, in which the neck is forced beyond its normal range and then snaps back. A neck sprain involves stretching or tearing of the ligaments and other soft tissues; a neck strain involves injury to the muscles themselves. Because the neck muscles control flexion (chin to chest), extension (looking up), rotation, and lateral bending (ear to shoulder), an injury to any one of them can severely limit head movement. Muscle spasms — the body’s protective, involuntary tightening around an injured area — frequently accompany a strain and can be as painful as the underlying injury.
Nerves exit the spinal cord in the cervical spine and travel down into the shoulder, arm, and hand, carrying sensation and controlling movement. When a nerve root is compressed — by a bone spur, swelling, or a fragment of a herniated disc — it can misfire, producing weakness, numbness, tingling, or radiating pain anywhere along the nerve’s path. Physicians refer to this pattern of symptoms as cervical radiculopathy.
When crash forces put too much pressure on a cervical disc, the annulus can tear and the nucleus pulposus can bulge or rupture outward — sometimes called a “slipped” or “ruptured” disc. If the herniation presses on the spinal cord or a nerve root, it can cause significant pain, numbness, and weakness. Chemical irritants released from the ruptured disc material can further inflame nearby nerves, which is one reason herniated disc pain can be so severe and persistent.
Higher-force collisions — including many motorcycle, truck, and pedestrian accidents — can fracture a cervical vertebra or injure the spinal cord itself. These are medical emergencies that can cause permanent paralysis or, in the most severe cases, death. Any accident victim with neck pain after a high-speed or high-impact collision should be evaluated immediately for a possible fracture or spinal cord injury before being moved, whenever possible.
